Search Provider

Search Provider can be used to find a provider based on any combination of the following search criteria:

  1. Provider NPI
  2. Any part of the Provider’s Address
  3. The Provider’s Category
  4. The Provider’s Specialty
  5. The Provider’s Business Name or First and/or Last Name (partial names are allowed)
  6. The Provider’s Group Practice Name
  7. If the Provider has TDD Capability
  8. If the Provider is an OPR Provider

Instructions

  1. Select the appropriate radio button to enable your desired search method.
    • If Provider NPI is selected, all other fields will be disabled. This field will become required.
    • If Provider Category is selected, you may search using address and provider criteria.
  2. The only required field when searching by criteria is the provider’s category. Provider categories are logical groups of providers such as Physicians, Audiologists, Chiropractors, Dentists and other individual providers. Provider categories can also be businesses such as Hospitals, Nursing Facilities, Pharmacies and other businesses.
  3. Once a provider category has been selected, the corresponding provider specialties for the selected provider category are refreshed and available for optional filtering.
  4. Select a number of results to display per page.
  5. Click Search Provider to return the results that match your search criteria.
  6. You will receive a message if no providers meet your search criteria. Otherwise, your matching results appear.
  7. Click Show Advanced Search to add additional criteria, such as TDD Capability (provider uses telecommunications device for the deaf), Provider Specialty 400 only (Ordering, Prescribing, Referring) and click Search Provider.
  8. Click Hide Advanced Search to close the additional search criteria.

Search Tips:

When searching for a provider enter as much information as possible. A provider’s City, County, or Zip Code is very useful in narrowing your search results.

The beginning part of a provider’s Business name or Personal name can always be used to narrow your search results.

If the Provider belongs to a Group Practice, the beginning part of the Group Practice name can be used to find all providers in that specific Group Practice.

It is sometimes helpful to change the Number of Results per page to a higher number such as 100 providers per page.

For Provider Categories that have a large number of providers, the Provider Specialty is useful for additional filtering. After selecting the Provider Category, click on the Provider Specialty drop down to select a specialty specifically for the selected provider category.

When searching by Provider NPI, your search is more precise and will typically return a smaller, more targeted set of results.
