Attention Providers Using the Authorization Criteria Function: Results that return prior authorization (PA) requirements are accurate. For results that return “There are no records found based on the search criteria,” there may be a PA requirement if limits have been exceeded. To verify PA requirements, please refer to the Medicaid Services Manual (MSM) Chapter for your service type at https://NevadaMedicaid.nv.gov/ and the Billing Guide for your provider type at www.medicaid.nv.gov.

Welcome to the Nevada Provider Web Portal!

Please read the following instructions carefully before proceeding to the registration process.
Important: There can only be one registered provider (administrative user) for each NPI/FEIN/zip code. This one administrative user must register as a “Provider” and will be responsible for assigning delegates for that provider.

This administrative user must have the following information in order to register:
  • First Name (Name associated with NPI facility/individual)
  • Last Name (Name associated with NPI facility/individual)
  • NPI/API
  • Tax ID (Federal Employer Identificaton Number(FEIN) or Social Security Number)
  • Zip Code
If you are registering as a delegate, you need to get a delegate code from a registered provider. The delegate must have the following information in order to register:
  • First Name (of Delegate)
  • Last Name (of Delegate)
  • Date of Birth
  • Last 4 digits of Driver’s License Number (DLN)
  • Delegate Code (obtained from registered provider)

Note: Each NPI and Delegate can register only once.
